A little preparation before you arrive keeps your check in smooth and helps you get the most out of your appointment time.

This page explains what to do before your visit, which forms you need, and what to bring with you.

Before Your First Appointment

To Help Our Team Start Your Visit On Time, Please:

  • Complete all new patient forms before you arrive
  • Confirm that Riverstone Urology is in network with your health plan
  • Ask your primary care doctor or referring specialist to send records if they are sending you to us

If you schedule by phone or through our patient portal, our staff will tell you which visit type fits your needs and whether any lab work or imaging should happen first.

If you cannot finish your forms at home, plan to arrive at least 30 minutes before your appointment so you have time to complete them in the office.

Insurance And Referrals

Riverstone Urology works with many major insurance plans.

Before Your Visit:

  • Call the number on your insurance card
  • Confirm that Dr. Russell Libby or Riverstone Urology Specialists is in network
  • Ask about copays, deductibles, and any need for prior authorization or referra
If your plan requires a referral from your primary care doctor or another specialist, make sure that referral is active for the date of your visit.

If you do not have insurance or choose to self pay, our staff will review self pay rates and payment options with you before your appointment.

You Get More Value From Your First Visit If You Arrive With A Clear Picture Of Your Concerns. Many Patients Find It Helpful To:

  • Write down your main symptoms and when they started
  • Note what makes things better or worse
  • List prior treatments you have tried and how well they worked
  • Bring any questions you want to be sure to ask
You do not need to prepare perfect notes. A short list on your phone or on paper is enough to keep the visit focused on what matters most to you.

If You Need To Reschedule Or Cancel

  • If you need to change your appointment, call the office as early as possible.
  • Early notice helps us offer that time to another patient who needs care.
  • Our staff will explain any cancellation or no show fees that apply under the office financial policy.
