First Visit Guide (Forms)
A little preparation before you arrive keeps your check in smooth and helps you get the most out of your appointment time.
This page explains what to do before your visit, which forms you need, and what to bring with you.
Before Your First Appointment
To Help Our Team Start Your Visit On Time, Please:
- Complete all new patient forms before you arrive
- Confirm that Riverstone Urology is in network with your health plan
- Ask your primary care doctor or referring specialist to send records if they are sending you to us
If you schedule by phone or through our patient portal, our staff will tell you which visit type fits your needs and whether any lab work or imaging should happen first.
If you cannot finish your forms at home, plan to arrive at least 30 minutes before your appointment so you have time to complete them in the office.
